All personnel at the practice have signed a confidentiality document and any breach of confidentiality will be dealt with in line with the practice disciplinary procedure. This may result in instant dismissal. Your health record identity is only given to a third party with your signed consent. If and when the CCG asks on occasions for data, this information is always anonymised.
