The practice runs a robust Long Term Condition system. Patients who have a long term condition are invited in for annual health reviews. Invitations letters are sent to patients on their birthday month and we ask that patients wait until they receive their invitation to book before they ring to book.   Our Reception team will book the correct appointments depending on the conditions you suffer from.

We also invite patients aged 40 to 74 who have no long term condition to attend for an NHS Health Check.  The NHS Health Check is a check-up for adults in England aged 40 to 74. It can help spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. Click on the link to find out more… NHS Health Check – NHS (www.nhs.uk)

The Practice Nurses and Health Care Assistants also run our Seasonal Flu Programme. The Surgery will be running joint Saturday flu clinics from Undercliffe Surgery from September and then  additional clinics at Brookroyd following this. Please don’t tie up the phone lines to reception with queries about booking your flu appointment. The Practice will contact eligible patients with an invitation to book once the clinics have been organised.
