How to register

If you live within the practice boundary you are eligible to register at the practice.

To register ask at reception for a registration form or download a form here. We also ask you to complete a new patient registration summary sheet that assists the receptionists to place you on the computer system.  You can download this from here:

If you are unable to open or use these documents please contact reception for assistance in registering with the practice.

You will need to attend the surgery with a form of identification and the Receptionist will also check that you reside within our boundary area.  Please note that it does take time to process your registration and this cannot be done immediately. If you still have a GP in the area you are still able to receive medical care and your repeat medications from them until your registration is processed.

Named GP

All patients at Brookroyd Surgery have a named, accountable doctor who is responsible for coordinating their care.  You can however see any GP or Nurse Practitioner at the surgery subject to availability.   Please ask at reception if you require more information.
